This woman had breast augmentation in the past. After finding a breast cancer on the left, she had bilateral mastectomies with removal of her implants and placement of tissue expanders on top of the muscle. She completed chemotherapy and luckily did not need radiation. Dr. Hubert then removed the tissue expanders and reconstructed both breasts with DIEP flaps. A clot in the vein of one of her breast flaps was detected in the recovery room. Recognizing this early, Dr. Hubert returned her to the operating room and salvaged the flap, using a small vein graft from her wrist, with an excellent outcome and no further problems. Nipples were made, and a small amount of fat transfer rounded out the final result. She declined tattoos for areolar reconstruction.
